FINANCIAL AND INSURANCE INFORMATION

PRIVATE PAYMENT

Payment is due at the time of service.  We accept major credit cards, cash and checks.

INSURANCE

We accept Medicare B; with a secondary insurance there is little out of pocket cost to clients. 

We are providers with several commercial insurance plans as well, such as Blue Cross and Blue Shield; United Healthcare, United Behavioral Health, and Optum. 

Many clients choose not to involve an insurance provider in their treatment for the following reasons:

*No diagnosis is required to be made.

*All information about you, including very personal information, cannot be obtained by the insurance company

*Your care is not dictated by insurance, but by the plan we develop together to meet your goals.

If you would like to submit information to your insurance company for reimbursement, we are happy to provide you with the documentation needed for out-of-network benefits.

RATES

• Initial session (60 min) $150

• Individual session (55-60 min) $150

• Letters of documentation or recommendation and completion of forms:  $100/hour (charged in 15 minute increments)

*A minimum of one week's notice is required for the completion of all paperwork.  Urgent requests will be billed at $200/hour

CANCELLATION POLICY

If you are unable to attend a session, please cancel or reschedule 24 hours prior to the appointment time.  If you cancel less than 24 hours in advance or do not show up for your appointment, you will be charged for the full rate of the session.
